Helping others find and maintain housing stability

Housing Stabilization Services is a Home and Community-Based Services (HCBS), a federal Medicaid (Medical Assistance) benefit program established in the state of Minnesota in July 2020 to help decrease barriers for people with disabilities, disabling conditions and seniors who are homeless or at imminent risk of homelessness to find, transition and maintain stable housing.

There are three types of services provided in housing stabilization services. These are:

REFFERALS:

Referrals for Housing Stabilization Services can come from the following service providers:

  • Targeted Case Managers
  • Waver Case Manager
  • Care Coordinators
  • Social Workers
  • Medical providers qualified to sign proof of disability
  • Self: person in need of Housing Stabilization Services
